tested, your template works by my side
I think that the problem is related to a plugin or module loaded in your page
try to disable the plugin "Page Builder CK - Contact" and tell me

ok, please check the plugin name : is it "contact" or "contact2" ? do you have both ?

I seem to have 2 plugins both named Contact, although the second one has an element name of Contact 2

ok, keep the Contact2
and remove (uninstall) the plugin Contact which is an old version
