I did something like that. And I received a message with two views. In the first position, the table but instead of the message only the names {fieldslist}
And below the message without styling

the tags {name} and others that you are using do not work. Only the tag {fieldslist} is working
The problem is that you can add many custom fields in your form and for now the ability to get the individual fields in your message is not implemented

You can use something like
[code]<div style="...">{fieldslist}</div>[code]

I add this on my (long ?) list to check how I can do that to use individual tags

the html is coded directly in the PHP file (helper) in the plugin "pagebuilderck" >> "contact2"

you can try to add a <style> tag and use the CSS selector to style the form. It will be added in the code of the email and this shall work
