Params not working

I installed Image Effect CK Params on a test site, but when I try to create a new effect, the interface is messed up and I can't do anytihing (see the image).

I checked the FireShield settings, but they are OK (Template Creator works well).
The site is lds.testsrp.it. Can you tell me what's going wrong?
If you need, I will send you the login name and password.
