Hi
if you have any doubt about the error page from template creator, you can use the one from the template Protostar and adapt it to your needs
you will then see that
1. + 2. are similar
3. you must then create a html page from scratch, this means that you must start with <html>.... and finish with <html>. This is a totally customized page ,nothing to do with joomla or template creator. Useful to put a big image with a link to the home
You must understand that hte error page in joomla is special, and has nothing to do with the other pages of your website. That is why the CSS and JS from the extensions are not correcty loaded, and template creator can not do anything against that
