Code started showing up on page (happened twice)

For some reason all the blocks on this page started showing raw code. I have the admin interface excluded in RSFirewall and it doesn't happen on other sites. What could the client be doing to repeat this.

something filters the code on save
check that all users that has the right to edit have "style" in the whitelist of joomla text filters (in the website config), or set to "no filter"
you can use the article versioning to get the previous working version of your article
