I tried using a menu item rather than calling the RSForm Module. I get the same result (the CSS custom code is still there). So I thing the problem is related to the form??
I will look further into the form

please, don't think about your menu item, nor the form, just read my request
- edit the module ID 142 (Maximenu CK)
- go in the "advanced" tab
- go in "module css suffix" option : do you have something here ? do you have "nav" written here ? if yes, then remove it
let me know

great that it works
I don't know why you have this nav suffix, because by default this filed shall be empty
this is not a bug, because you can add any suffix value here, the only problem is that this css class is used in Bootstrap 4 loaded by RSForm and that modify the behavior of the module area
